BOOKING POLICIES: Your Initial Consult Appointment is a significant amount of clinic time, which is reserved for you to see the Physician, a Financial Counselor, and the Clinical, IVF, and/or the Third Party Reproductive Care Coordinator.

For this reason, patients are required to make a deposit of $125 to book/schedule their New Patient Consultation Visit. This deposit is applied to your New Patient consultation and any services performed at that time. See additional information below specifically related to both CASH and INSURANCE coverage patients.

Patients who “no-show”, fail to cancel or reschedule their initial consult appointment, with less than 72-hours (3 business days), are assessed the $125 deposit as a no-show/cancel fee.

Our office accepts Visa or MasterCard credit card payment over the phone. Patients are required to provide this information at the time of scheduling the New Patient Consultation with our office.

CASH PATIENTS: Your New Patient deposit is credited toward your consult and services for this visit.

INSURANCE PATIENTS: Our office only files claims for covered services according to your individual policy/plan benefits. You will be credited or refunded any deposit amount less your required co-payment, cost share, and/or applicable deductible amount due, for your New Patient consult and services for this visit.
